Position Information:
The Team Leader, RN is responsible for the coordination of the departmental clinical operations for home health with primary focus on home visits. Coordinates admissions/referrals to facilitate timely patient flow throughout the department. Delegates patient care to appropriate personnel by making assignments based on needs, abilities, and expertise. The Team Leader understands the needs of the organization and supports the mission, values, and management of patient-care services. The Home Health Department provides full service home health to patients in Wake County and some of the surrounding counties.

Experience Requirements:
2 Years Clinical – Home or Community Health Required – And 1 Year Leadership Preferred

Education Required:
Graduate Nursing Required – And Bachelor’s Degree Nursing Preferred

Licensure/Certification Requirements:
Registered Nurse Required

Hours of Work:
12 noon – 8 pm m-f

Weekend Requirements:
As Needed
